  Step 1: Contact and Participant Information
   
  Once your application has been submitted, it will be reviewed and you will be contacted regarding your eligibility for the program. If approved, your level of subsidization will be noted in your account in our registration system. Approval for an application form is valid for one year from the approval date.

  If "Yes", please email the following documents to subsidy@whitby.ca: Ontario Works cheque stub or ODSP or proof of Whitby residency (Driver's Licence, Passport, Rent Receipt etc.)

If "No", please email the following documents to subsidy@whitby.ca: Proof of Whitby Address, most recent combined family household Income Tax Return Notice of Assessment(s).
   
  Step 3: Approval
   
  - If approved, each member of your family will be entitled to take one program run by the Town of Whitby each session (Fall, Winter, Spring and Summer) for a total of 4 programs a year.

- If approved, the Town of Whitby will cover 50% or 100% of your program fee up to a maximum of $150 per person, per session.

- If you are eligable for 100% subsidization and the program you choose is more than $150, the Town of Whitby will pay for $150 of that program and the balance will need to be paid by you. (We accept Cheque, Debit, Cash, Visa or MasterCard).

- Eligibility for subsidization is good for one year from the date of approval

- After one year, you need to re-submit an application with attachments in order to be considered for further subsidization.

- Seniors' Programs, Children's Programs, Adult Programs, Health Club Memberships, Aquatic Memberships, Aquatics Passes, and Aquatics Tickets, Fitness Classes, Fitness Passes, and Fitness Programs that are held at Town of Whitby locations are covered under the Town of Whitby Access Program.

- Town of Whitby locations include: Whitby Civic Recreation Complex (CRC), Iroquois Park Sports Centre (AOP), McKinney Centre, Whitby Senior's Activity Centre, Heydenshore Pavilion, Cullen Central Park, Centennial Building, and Whitby Town Hall.

- Information about Programs can be found in the Town of Whitby Activity Guide, or online at www.whitby.ca
   
  What is not included:
   
  - The Town of Whitby will not cover more than one program per child/adult per session.

- Programs that are not run by the Town of Whitby (Community Connection, Station Gallery).

- Funding coverage over and above $150 per person per session.
